When I graduated in medicine, and chose psychiatry as a specialty, there weren't many prospects for a job that could be considered health. The goal of most was to be a psychoanalyst, to do analysis, and, in this line, the top was to become a trained psychoanalyst. As another friend, Antonio Lancetti, said, there were times when being anti-asylum was not fashionable.
